Pardon my ignorance, but how does the bad code get put there in the first place? Without that being blocked, editing it out won't last long. Does this apply mostly to people who host their site on their own machine, or does it apply to sites hosted by big-name ISP's?

There are a number of different ways but essentially it comes down to poor security by the users who maintain the sites. Things like not changing default passwords, updating software or only trouble shooting web page errors until they work and not making sure they are secure. The bad people send out bots to try access websites through one of many known issues and insert the malicious code that way.
